The articles by Journal of the American Medical Informatics Association discusses a survey that was conducted on 323 patient and 389 care partners at Geisinger Health System to gauge the inability of the patient to get access to the health information aimed to resolved the problem of health literacy among the patient. The selected sample defined that 0.4% of registered adult patient use the health portal to share their information. Kraftson, R. H., Kraftson, M. O., Mudditt, M., Schrogie, J. J., & Simon, W. H. (2000). U.S. Patent No. 6,151,581. Washington, DC: U.S. Patent and Trademark Office. In order to develop an effective visit notes plan for the patient, a well design data collection system ought to be established. The data system generated will ensure that the available patient data will be accurate to enable the organization to understand their client needs. The U.S have been initiating trials since 1966 such as (Provision Application 60/033,099) up to date trying to renovate an application that would record and process population data especially the physician clinical and patient information for a survey to facilitate the management of this information. If such innovations succeed, an organization may request the information to use it to improve the patient visit notes since it will provide a wider information about various patients diagnosed in the country covering a substantial period of time. However, the system for information acquisition and the processing of the received data have been termed as compromising due to prevailing made up data of physician and patient ways to receive both the clinical information and patient satisfaction information respectively (Kraftson et al., 2000) Mafi, J. N., Mejilla, R., Feldman, H., Ngo, L., Delbanco, T., Darer, J., ..& Walker, J. (2016). Patients learning to read their doctors’ notes: the importance of reminders. Journal of the American Medical Informatics Association, ocv167. The American Medical Informatics Association explains an article on whether patients who are invited to review their clinical notes are motivated to continue accessing their health data or not. After the researcher did an analysis on the OpenNotes trial participants in the past two years at the Beth Israel Deaconess Medical Center (BIDM); and at the electronic alert patient showed that though millions of patient countrywide continue to gain the access to clinics notes, still this notification through emails are sustainability ignores by most of the respondents. Hospital records notification can be an effective strategy in the plan if only other practical measures could be involved such as introduction of notification through text messages which will be assured that all patient are able to access the information direct from their mobile phones (Mafi et al., 2016) Wright, E., Darer, J., Tang, X., Thompson, J., Tusing, L., Fossa, A., ... & Walker, J. (2015). Sharing physician notes through an electronic portal is associated with improved medication adherence: quasi-experimental study. Journal of medical Internet research, 17(10). The Journal of medical Internet research article on quasi-experimental survey showed that after patient has accessed health services, offering health web portals in their respective health care providers which enable them to access their primary care physicians’ (PCPs) notes improves medical adherence (Wright et al., 2015) Walker, J. (2015). Open visit notes: a patient's perspective and expanding national experience. Journal of Oncology Practice, JOP-2015. Some of his efforts include the creation of a website where his patients can comment on different issues regarding their health progress after receiving their treatment. One of his patient (D.dcB.) has become active in the world patient engagement which was facilitated by the motivational health team from Martin clinic. The two introduced initiative which include the patient engagement toward transparency have a substantial strategies that can be used to facilitate the visit note plan (Walker, 2015). Walker, J., Meltsner, M., & Delbanco, T. (2015). US experience with doctors and patients sharing clinical notes. Bmj, 350, g7785. As Jan Walker, Tom Delbanco and Michael Meltsner explained on the US OpenNotes initiative spurred by searching the appropriated demonstration to improve online patient clinic assess showed that after the launch of this initiative a significant number of health providers had accepted this offer. In the past decades, health providers have tried their level best to improve clinics records availability, with the help of electronically powered methods, now health facilities are able to file the information where their patients can access this information without necessarily visiting the health centers. Such imitative which enforces online health data filing will contribute substantially to improving visit note plan (Walker, Meltsner, & Delbanco, 2015). Jackson, S. L., Mejilla, R., Darer, J. D., Oster, N. V., Ralston, J. D., Leveille, S. G., ... & Elmore, J. G. (2014). Patients who share transparent visit notes with others: Characteristics, risks, and benefits. Journal of medical Internet research, 16(11). The process of inviting the patient to participate in reading visit notes will improve the sharing of information with their health care providers. As sharing private health information becomes difficult due to the magnitude of essential personal information its retains, opening health provision web site where patients who are willing to share their health information with other people may contribute to the psychological acceptance of one's health condition. According to the Journal of medical Internet research, patients who feel free to share their health information with their family members are likely to adapt and recover from their sickness rather than those who only keeps the health conversation with their doctors (Jackson et al., 2014). Oster, N. V., Jackson, S. L., Dhanireddy, S., Mejilla, R., Ralston, J. D., Leveille, S., ... & Elmore, J. G. (2014). Patient Access to Online Visit Notes Perceptions of Doctors and Patients at an Urban HIV/AIDS Clinic. Journal of the International Association of Providers of AIDS Care (JIAPAC), 2325957414526783. People living with HIV/AIDS are among patient who faces a lot of challenges in the societies especially while securing their medical information. Inviting people with such special cases of diseases to read their doctors visit note in a secure manner through electronic portals will improve provision health measures hence improving their health conditions. In a one year analysis of post -intervention data from 819 patients and 99 doctors report findings showed that HIV patient and physicians are ready to share their information online rather than direct/ face to face interventions. The reason being that the exchange of information online is convenient where the patients are not required to pay a visit to the HIV clinics. In addition, it helps to secure personal health information through minimizing the number of visits that HIV patients have to conduct annually (Oster et al., 2014) Pell, J. M., Mancuso, M., Limon, S., Oman, K., & Lin, C. T. (2015). Patient access to electronic health records during hospitalization. JAMA internal medicine, 175(5), 856-858. According to Institute of Medicine, 2001 recommendation on the patient engagement in the provision of health care information facilitated transparency within the patient and his/her relatives. The introduction of Electronic health records (EHRs) enhanced the Institute of Medicine objectives through the creation of patient portals. While providing the outpatient a web portal will ensure that proper enrichment of health information to the concerned patient is efficient. However, such implication to the inpatient have not been implemented, but most of the health provider argue that the inpatient have direct access to health providers, and provision of HER would lead to confusion. The research analyzed in this article show that HER does not bring confusion for inpatient or either increase workload wherein the resultant procedure can facilitate a more positively respondents of patients which facilitated the easy accesses of patient to their health information hence understanding their disease progress (Pell et al., 2015) Bruno, M. A., Petscavage-Thomas, J. M., Mohr, M. J., Bell, S. K., & Brown, S. D. (2014). The “open letter”: radiologists' reports in the era of patient web portals. Journal of the American College of Radiology, 11(9), 863-867. The nature of the information released on the patient portals ought to be investigated by the healthcare providers to minimize chances of incorrect readings the results. For instance, the X-ray results are among the lab results that may have this implication. Though historically radiologist official recorded, reports have been communicated to the patient, the access of this information may raise other issues where the patients may misread the information and relate to the possible disease symptoms which may not be the case. In this scenario, the information posted on the patient page should be assessed first to ensure that patients are not exposed to some of the compelling data that would compromise their treatment. While designing an effective plan to initiative easy access of patient to the visit notes, therefore, the information limits should first be set (Bruno et al., 2015) . References Bruno, M.
